HL7 SANER Relay Network

Symptomatic has been working with HL7 International, MITRE, and the CDC in developing an open-source javascript implementation of the SANER Implementation Guide (Situational Awareness for Novel Epidemic Response).

Our solution incorporates a SMART on FHIR data mining utility (Measure Producer), that can query hospital EHR servers for COVID19 data.  This allows it to be compliant with CARES Act Section 18115 requirements, in that we are already querying for Covid19 antigen LOINC codes, and have the capability to query and report on medication and PPE supplies, staffing levels, and other pandemic related activities that are available via FHIR but outside the scope of traditional LIS and public health reporting networks.

Our solution is running on AWS with a docker infrastructure, using Mongo Atlas.  As such, we've secured most HIPAA compliance aspects, and the platform is ready to be deployed to EHR app stores (Epic App Orchard; Cerner Code), where it can then assist in forklift upgrading hospitals across the country, by providing additional data-mining and automated reporting functionality.

In May and September of this year, we participated in the HL7 SANER connectathon tracks; and have validated the solution with CMS/MITRE testing servers (MeasureProducer, MeasureReceiver, Intermediary), as well as the CDC National Healthcare Safety Network FHIR servers (MeasureReceiver).  We are currently pulling data from both CDC and HHS websites.  

Any design suggests or feedback would be much appreciated.   We're currently looking for pilot sponsors to assist in the rollout via the EHR app stores.   Able to easily add features and build custom modules, as needed.

What type of pilot sponsors are you looking for? Would a school using Binaxnow antigen tests, recording results in the their local school system be able to use an EHR app to have their results and student demographics queried to report to state and federal ? What modifications, if any, would need to be made in the school registration system for the app to do the query? Also, I tried to view the output images, but the type is to blurry to read.

Well, preferably a pilot sponsor with some budget. This stuff is expensive to host and develop!

But financial matters aside, a school would be a fine place to do a pilot. If we’re using a screen scraper, then I doubt any adjustments to the registration system would need to be made.

We wouldn’t be able to use the data-mining utility, of course. That is an EHR utility. But if the school had some sort of nursing EHR, then it would be a matter of whether it’s been upgraded to include FHIR Interoperability functionality.

That answers my question. The data mining utility requires an EHS. The "secure scraper" is just a concept to avoid requiring a particular platform. Thank you for this information. It sounds like the data miner would work well in traditional healthcare settings.
